By signing this Consent, the Data Subject confirms that he / she has been informed of his / her rights and obligations under Act no. 18/2018 Coll. on the Protection of Personal Data and Amendments to Certain Acts (hereinafter referred to as the “APPD”) by the company ableneo Slovensko s.r.o., company ID number: 47 002 140, with registered office at Vysoká 26, 811 06 Bratislava (hereinafter referred to as the “Company”) and its affiliates ableneo Česká Republika s.r.o., company ID number: 05173167, prior to the obtaining of his/her personal data and that the Company has in advance communicated to the Data Subject the information pursuant to Section 19 of the APPD, which are as follows:
- a) the identification and contact details of the controller,
- b) the contact details of the data protection officer,
- c) the purpose of personal data processing to which the personal data are intended, as well as the legal basis for the processing of personal data,
- d) the period for which the personal data will be stored,
- e) the right to request from the controller access to personal data relating to the Data Subject, the right to rectify personal data, the right to delete personal data or the right to limit the processing of personal data, the right to object to the processing of personal data as well as the right to the portability of personal data,
- f) the right to revoke his / her consent at any time,
- g) the right to file a motion to initiate proceedings under Section 100,
- h) the information whether the provision of personal data is a statutory or contractual requirement, or a requirement necessary to enter into a contract, as well as whether the Data Subject is obliged to provide the personal data and of the possible consequences of failure to provide such data.
By signing this Consent, the Data Subject also confirms that the personal data provided are correct and complete, the provision of the data is voluntary and at the same time provides his / her explicit consent pursuant to Section 14 et seq. of the APPD to the processing of the so provided personal data by the Company for marketing and advertising purposes which associated with sending of newsletters.
The Data Subject hereby provides his / her consent, in particular, to the obtaining, provision, disclosure and storage of personal data provided by him / her to the Company for the above-mentioned purpose of keeping records and processing of personal data.
The scope of personal data which the Company is authorized to process in order to achieve the purpose of processing of personal data: name and surname, job position, e-mail address, telephone contact.
The personal data of the Data Subject stated in this Consent will be neither disclosed nor forwarded by the Company to any third parties.
This Consent to personal data processing is provided by the Data Subject for a period of 3 years.
This Consent may be revoked vis-à-vis the Company in writing at any time by sending a consent revocation notice to the registered office of the Company, or by e-mail to info-sk@ableneo.com.
